Alternatives to Knee Replacement: Non-Surgical Options Worth Knowing

July 5, 2026  ·  6 min read  ·  FAQ

If an orthopedic surgeon has started the knee-replacement conversation and you aren't ready, you aren't alone. Replacement is a remarkable operation when it's the right answer, but plenty of people want to exhaust other options first, or aren't good surgical candidates for medical reasons. Here is an honest look at what else exists.

First, the standard non-surgical steps

  • Weight management. Even modest weight loss meaningfully reduces load through the knee.
  • Targeted strengthening. Building the muscles around the knee, especially the quads and hips, offloads the joint and reduces pain for many people.
  • Activity and footwear adjustments. Lower-impact movement and supportive shoes keep you active without flaring the joint.
  • Injections. Cortisone can calm a flare; results are often temporary and tend to fade with repeated use.

When those have run their course

Many people reach a point where injections help less each time, therapy has plateaued, and the next thing offered is surgery, but they aren't ready or able. That gap is where it's worth knowing the joint pain in osteoarthritis is driven heavily by inflammation in and around the joint, not only by the worn cartilage itself.

Where low-dose radiation therapy fits

Low-dose radiation therapy targets that inflammation with very small, precise doses, far lower than cancer-treatment doses. The goal is to quiet the inflammatory environment in the joint so day-to-day pain eases, regardless of how worn the cartilage looks on imaging. It is non-surgical, painless, requires no downtime, and is delivered over a short series of brief visits.

Published European series report meaningful pain relief for a portion of patients, including people with advanced, bone-on-bone arthritis. It is not a fit for everyone and individual results vary. Importantly, choosing it first does not affect your eligibility for replacement later if you decide that's the right step.

If you've been told you need a knee replacement and want to understand the non-surgical options first, that's a reasonable conversation to have.
